I authorize The Morgan Adams Foundation to release my child's photo and/or interview to the news media or public, as well as all future marketing materials including the organization’s website. Such information can include age, city of residence, and personal health information including diagnosis, if applicable. Furthermore, I do not expect to receive financial compensation or consideration of any kinds for use of the story. I understand that the story and related materials are the property of The Morgan Adams Foundation.

Note: You may revoke this authorization in writing at anytime, but once a news outlet has obtained footage, photographs, health information or interviews, it cannot be rescinded from that point forward. You have the right to inspect or copy the information to be released to The Morgan Adams Foundation and The Motorized Madness event. The Morgan Adams Foundation will consider this consent valid for 10 years unless specifically revoked in writing by the parent, legal guardian or person over the age of 18. You may refuse to sign this authorization.
